Thank you for taking the time to fill out our Interprofessional Continuing Education (IPCE) intake form. The purpose of the form is to evaluate potential accredited education activities in order to prioritize IPCE offerings and to align with the mission of Intermountain Healthcare. 

Following completion of this intake form, an IPCE team member will reach out to you to conduct a more detailed interview concerning your proposed activity. Please remember that for most conferences, there is a 9-12 month timeline for adequate marketing, planning, and execution. If your activity is approved, you must fill out a CE application which requires a finalized agenda and needs assessment documentation. If your application is submitted less than 90 days from the activities date, the request will be declined. Thank you for understanding.
